At LRCS we value the health and fitness of all our students and staff. Our school health program aims to assure students are healthy, safe and ready to learn, while connecting students and their families to health resources in the community.

EMERGENCY HEALTH PLANS 
Students with a life threatening condition are required by law each school year to have their doctor’s orders prior to starting school or sports. Medical forms can be found HERE. 

DOES YOUR CHILD NEED MEDICATION AT SCHOOL? 
Parents/guardians are encouraged to administer medication at home whenever possible. If it is necessary to administer medication(prescription or non-prescription) during school hours, a medication administration permission form must be completed by a licensed health care provider and signed by both provider and parent. Medications must be in original labeled containers and transported to school by an adult. TDAP
If your student is entering 7th grade they are likely due for another TDAP (tetanus, diphtheria, and pertussis) vaccine. Please review your child’s immunization records prior to the start of school. 

IMMUNIZATIONS 
Immunizations are required for all students enrolling in public school. Immunization requirements vary depending on age and grade level. Contact your child’s health care provider or the school nurse if you have questions regarding your child’s immunization status. Additional information about immunizations can be found HERE.

EMERGENCIES:
It is IMPERATIVE that parents/guardians make certain the school office has up-to-date information on where parents/guardians or a designated emergency contact may be reached throughout the day in case of emergencies. Please notify the school any time a home or work phone number changes for you or your emergency contact.
